All crew on board an aircraft departing from a port from Trinidad and Tobago must produce: - Valid passport/travel document, Completed Immigration E/D Card (Form 2).
  A pleasure craft/yacht departing from a port in Trinidad and Tobago to a foreign port must produce: - General Declaration (Departure Report) Form 5.   Fees for CARICOM and Non-CARICOM Nationals.
   
Seaport Departure Requirements for Vessels

Duly authorized shipping agents in Trinidad and Tobago are allowed to represent and conduct business for and on behalf of ships owners/operators/crew.
A ship departing from a port in Trinidad and Tobago to a foreign port must produce: -
Notice of Departure Form 16
Grant of Immigration Clearance Form 27
General Declaration (Departure Report) Form 5
Crew List
Passenger/ Intransit Passenger List

Crew Departing on a Vessel
All Crew on board a ship departing from a port in Trinidad and Tobago must produce: -
Valid passport/travel document
Completed Immigration E/D Card (Form 2)
Appear before an Immigration Officer if required so to do
Any change of crew on or off a vessel must be reported to Immigration Authority for approval prior to such change being effected.
