Passport Application Forms can be obtained from any Immigration Office in Trinidad and Tobago, via TTConnect or at Trinidad and Tobago Foreign Missions. (Call to check hours of availability).

Complete Passport Application Form – for adults 16 years and over

Complete Passport Application Form – for minors under 16 years of age

The maximum period of validity of ALL passports is five (5) years from the    date of issue.
Provide a Police Report

An Incident Report form from the Police Station in the Applicant’s district stating that he loss was reported.
Provide a Statutory Declaration

A Notification form for a Lost, Stolen or Mutilated Passport, which must be certified by a Commissioner of Affidavits or Justice of the Peace.

Provide a Fire Report (if applicable)

In cases of destruction by fire, a certified report from the Fire Authorities would be required in lieu of a Police report.

Proof of Trinidad & Tobago Citizenship

All documents which were submitted with previous application (Original Birth Certificate etc)

Provide One Passport Photo
  NOTE: Vending machine photos are not acceptable.
Please do NOT staple, fasten or glue your photos to the application form.
It is to be noted that the reported Lost, Stolen or Mutilated Machine Readable Passport will be de-activated upon submission of the relevant forms and as a result would no longer be valid for travel.
